Field Placement Program Notice of Intent Form

This form must be submitted by Oct. 4, 2013 for Fall field placements. (Note: Late submissions are discouraged, but may be considered on a case-by-case basis).

Application and Registration Process

1. Review the Directory of Placements and resources on Sakai.
2. Submit your notice of intent form.

The Fall application deadline is Oct. 4, 2013.

The Spring application deadline is Nov. 1, 2013.

Note: Students who are on probation may not participate in a Field Placement. This form is hosted by a third-party vendor. Please review the Pacific McGeorge privacy policy.
